Evaluating a Weight Management Program in a Primary Care Setting
Project Description: The aim of this Quality Improvement (QI) project is to evaluate the impact of a comprehensive weight management program at Archer Family Health Care. The setting is a nurse-led integrated primary care and mental health practice in Archer, Florida. Evidence based protocols have been developed for implementation as part of an initiative to improve health in high risk patients. This project will serve to evaluate if a structured weight management program improves clinical outcomes. It will also serve to identify patients with comorbid conditions such as diabetes, hypertension, depression and binge eating disorder.
